Dear Dr.Altmann. My daughter, Jasmin Kees, was at patient at your practice until 2004(until she was 6 years old). After that I moved back to Britian. I do not have any medicial records from my daughter whilst we lived in Germany. I particulary need to know what immunisations she had and when she received them. If you only have them in German our doctor here will translate them but if you have the notes in English that would be great. Could you, there for, please send Jasmin's medical notes to me together with her immunisation record. Our previous address was Weiler Strasse 9, Fischen, 87538.
